Unable to request quote via web page

I am unable to submit a request for a quote via the web page. My employer will not accept a quote without our firm explicitly stated on a quote document. When I complete and submit the “request a quote” form under “contact us”, I get a blank screen. There is a note that says “check your browser settings (such as incongnito)”, but I have no idea which of the many settings to check. Calling the phone number for the US HQ directed me back to the website.

I have tried different browsers and different computers logged in under different credentials. Stil no luck.

Any suggestions?
